Short Term Rehabilitation

Four Chaplains Nursing Care Center is a bridge for individuals leaving the hospital, but not yet ready to return home. We provide an opportunity to rest, recover and rehabilitate.

We are deeply committed to providing the highest quality rehabilitation services. Our Center’s rehabilitation services include physical therapists, physical therapy assistants, occupational therapists, speech therapists and restorative nursing staff. Individualized rehabilitation plans ensure our patients reach their maximum potential and return home as soon as possible.

Our rehabilitation programs are designed to treat a wide range of illness and injury, such as:

  • Stroke
  • Amputation
  • Joint replacement
  • Fracture
  • Spinal cord injury
  • Brain injury
  • Cognitive and perceptual retraining
  • Surgical recovery
  • Heart disease
  • Lung disease
  • Alzheimer’s disease

Physical Therapy

Physical Therapy focuses on helping individuals regain strength and coordination, and on promoting proper body mechanics to improve function and mobility.

Treatment goals include:

  • Learning efficient and safe transfers (such as from bed to chair)
  • Increasing range of motion
  • Restoring, stabilizing and improving mobility
  • Learning to use canes, walkers and wheelchairs, when necessary

Occupational Therapy

Occupational Therapy emphasizes self-care and the performance of daily activities.

Treatment goals include:

  • Upper-body strengthening
  • Energy conservation
  • Restoring, stabilizing and improving dressing and bathing abilities
  • Regaining balance
  • Safety awareness
  • Development of light home management skills such as laundry, cooking and cleaning

Speech Therapy

Speech Therapy addresses hearing, language, communication and swallowing disorders.

Treatment goals include:

  • Improving speech-language expression and comprehension
  • Refining oral motor skills for more accurate speech
  • Restoring, stabilizing and improving swallowing
  • Developing strategies for safe eating
